Bank of America will be retiring the Bank of America Healthcare Payments
   application in the third quarter of 2007. The processing of payments will be
   migrated to Bank of America's PayMode^® platform. To ensure continued
   receipt of electronic payments, you must enroll in the PayMode service.

    PayMode offers the following benefits:
    · On-line reports and claim download options eliminating the need for paper
      claim data sent from HealthLink. Your EOB statement will be delivered through
      the PayMode website. No more waiting for USPS delivery.
    · Detailed history of all PayMode payments from HealthLink at your finger tips
      on the PayMode website.
    · No changes to your bank accounts and no special software to install.
    · The on-line enrollment takes less than 10 minutes.

    HealthLink will begin making payments to you electronically once you have
    enrolled and have been activated in PayMode.
